I experienced considerable wasted time without the "Proof of Interlock Ignition Installation" which was not sent to the service provider. I phoned CS and was told that an electronic copy would be sent to the MVD. MVD declined Intoxalock procedure. The service provider was very knowledgeable, helpful, patient, and gracious with service at Meistrells Mobile Electronics at 1429 N. 17th Drive, Phoenix, Arizona 85007. I highly recommend Meistrells. I give the service provider 6 gold stars.
