Our taxes were filed incorrectly after the accountant added "estimated payments" to our return without any verification or proof. As a result, we ended up owing tax and incurred interest/penalties. I was not aware of this until I was contacted by the IRS and had to personally notify H&R Block of the mistake to which they offered no solution.
