Very unprofessional. First and foremost, I was sent new patient paperwork pre-filled with another patient's information (HIPPA violation) then they wait until the day of my appointment (an hour before) to tell me that I will need to reschedule because they didn't receive my completed patient paperwork which it was sent days in advance with proof then pushed my appointment even further out. As a healthcare provider, I'm very disappointed in this practice and wouldn't recommend!
