Easily applied, was contacted by Dr. David had a phone call and was able to obtain my card.
There were lots of available slots to make appointments including the evening. It would have been helpful to know that before you could see the doctor you had to fill out a bunch of forms that took longer than the time before my appointment. Then I couldn't figure out how to get on to the line, but the doctor called me back and couldn't have been nicer or more helpful. By the end the whole process was quick.
