My first appointment with Kevin at FORM Physical Therapy was wonderful, and everyone was very friendly and welcoming! I had previously seen Kevin Elavia for TMJ, and working with him truly changed my quality of life. I went from barely being able to open my mouth, to now having complete range of motion of my jaw.
