I was at Elite Eyecare at 3:30pm at the Grand Concourse location and was seen by Dr. Li . Your staff refused to provide me with his full information. I paid for an eye exam to get contacts. After I was provided with a trial pair, I asked for my prescription and was then told I was required to pay $25 to hand over my RX numbers, which explicitly goes against Part 315, specifically 315.3 of the Federal Trade Commission policy, codified at 15 U.S.C. 7601-7610 under the Fairness to Contact Lens Consumers Act.
