We purchased one prescription glass for my son first from Costco optical , Stanford Ranch Road. We ordered the frame and paid for it but they only issue small receipts without the itemized receipt. Subsequently we purchased the lens after eye test. Both eye test and frame purchases claim to Blue Cross Blue Shield are denied due to lack of information on their receipt.
