the doctor Joseph Alvarado. will hold your career hostage until you meet his needs.....like forcing you to take a sleep apnea test....( it is NOT the law). I don't care for him, personally I think he gets kick backs from the sleep apnea test centers. I'll be calling the Arizona medical board and filling a complaint against him.i highly suggest if you have a CDL don't go here for a physical.
