If I could leave this establishment zero stars I would. I am a prior employee and trust me when I say, you do not want to work for this company nor do you want your loved ones here. They have withheld my last paycheck and refused to rectify the issue. The employee's (Pat Flores) excuse for not paying me or even attempting to help me was because she just did not have time to ensure I got paid because the actual employees were more important. Earlier this year, my sister spent most of her final 4 months of life in Centre Avenue Health and Rehab, from the rehab floor to long-term nursing to hospice. In prior years, she had also been a patient at Centre; especially during the pandemic, the facility seemed understaffed, the employees acted stressed, and care was not optimal.
