I called and they told me to go to the bank and get it. I stopped at 2 branches they did not have the form.
Went in to get cash...my receipt reads I was there at 11:25 AM on 7/3/2025 Dominique waited on me...she was RUDE..RUDE...RUDE..she was mouthy with me..she was working out of cash box 8...I will NEVER go back to that branch EVER...it is little wonder this branch has such a low rating and most reviews is about their rudeness...hope the branch manager reviews this and has a talk with her...it is on your security camera
Very polite and professional makes banking with Chase that much better I look forward to seeing the staff
