I booked an appointment with Notary Armita Mossalanejad for a travel authorization for my child to bé able to join me to France accompanied by his mother. On the phone, I was told that any notarized declaration would be acceptable. However, once in front of Armita Mossalanejad (who prominently displays a PhD title :)) , she informed me that it could only be done using the official Canadian government form, which she downloaded in front of me.
